If you would like to save time at your visit, please fill out your New Patient Paperwork and bring it with you to your appointment. Don't fret if you don't get it done - we'll be happy to cover all the bases here in our office as well!
If you have dental insurance, please bring your insurance card, along with the policyholder's social security number and birthdate with you as well.
