My Bracket Came Loose! What to Do in an Orthodontic Emergency

2/28/2025

Orthodontic issues can be uncomfortable, but they’re rarely urgent. However, knowing how to handle them can save you from unnecessary discomfort and treatment delays. Here’s what to do if you face a braces-related problem:

When to Call the Orthodontist
If your braces or appliance is causing significant discomfort, breakage, or other issues, contact our office. Some issues can be resolved over the phone or by sending pictures, while others may require an in-person visit. If you’re unsure, don’t wait—call us to avoid complications or delays in treatment.

Common Orthodontic Issues and Quick Fixes
  1. Loose Brackets, Wires, or Bands: If a bracket comes loose, use sterile tweezers to slide it back into place. If it rotates and causes irritation, temporarily secure it with relief wax. Call us to schedule a repair.
  2. Protruding Wire: Use a Q-tip or pencil eraser to push the wire flat against your tooth. If that doesn’t work, cover it with relief wax. As a last resort, carefully snip the wire with sharp clippers, but be cautious not to swallow the clipped piece.
  3. Ligatures Come Off: If a rubber ligature comes off, you might be able to put it back with sterile tweezers. If it’s a wire ligature, either tuck it back with a Q-tip or remove it entirely. Contact us to determine if you need a visit.
  4. Discomfort or Soreness: It’s normal to feel discomfort after adjustments. Stick to soft foods and rinse with warm salt water for relief. If the pain persists, contact us for advice.
  5. Mouth Sores and Irritation: Braces can irritate your cheeks, lips, or tongue. Apply a small amount of relief wax to the area causing discomfort or use a topical anesthetic like Ora-Gel for pain relief.
  6. Food Caught Between Teeth: This is not an emergency but can be uncomfortable. Use dental floss, an interproximal brush, or a toothpick to remove the food.
  7. Piece of Appliance is Swallowed: This is rare but can be alarming. If coughing or breathing issues occur, seek immediate medical help. If you suspect the piece was swallowed, contact us for guidance.
